Really shouldn't be taken literally. NSFW is roughly equivalent to PG13 and above.
Used on Fark a lot too (not sure where it actually originated) - a "Boobies" link to a picture gallery marked SFW usually just means there's no nipple, your employer is still unlikely to be amused by your browsing galleries of scantily clad women on work time.
