I recently had a colonoscopy. During my first visit to the doctor, a British medical textbook was in the room and I thumbed through it. When he came in, he took the book and described what was happening in several pictures on several pages.
One of the points he made more than once was that it's way too common for people to "literally try to crap out things that are attached." He strongly advised to, as the old saying sort of goes, poop and get off the pot. And don't read; the act of sitting on the toilet creates a strain that shouldn't last any longer than necessary.
Much of that was news to me and sort of begins to answer the question posed at the beginning of this thread.
