I think it depends on whether she asks what you want for your b-day. If you say "no big deal" or a "blow job" makes a difference. It also matters what you do for her on her b-day.
Seriously - it's a day in the week. If she chooses to make it extra special for you, enjoy it (as you should do for her). You're not 8 anymore, and if you can't talk about these things, give up.
