I’m not a big fan of peeing in a line up at sporting events and sometimes I can’t relax enough to, but I can have sex in front of other people no problem, so the two may well be not related.

This makes it sound like sex is very dirty to you. You need to get your head straight a bit on this and find a girlfriend willing to help you out with it. If you can get off alone then its not a physical thing. Also are you attracted to her? Perhaps the problem is she doesn’t really turn you on at a physical level.
