Even with cable, though, there are differences. We had TimeWarner digital before we moved; good price, all the premium channels, an excellent onscreen guide, and decent selection of channels.
Since our move, we now have RCN digital. They have a better selection of channels, but they're more expensive and their remotes are finnicky, plus their onscreen guide isn't all that great.
I've never had a dish, so I can't say how they are.
