Letting my Shenmue save game get in the hands of my little sister. I was trying to get all the stuff and I let her play some (I thought she would use the save I created for her.) I come back later and my game is completely gone... I haven't played the game since...
Also, I remember quicksaving as I died in Jedi Knight 2.
