Nobody should let themselves, their friends or family get to that point, even if you have to put a gun to their head to make them get up. She has herself to blame for getting that fat but her family and friends are to blame for letting it continue on until this point.
They should be ashamed of themselves.

So um. . . .I take it your life is perfect then, or should we be equally heartless when you have a problem or twelve?
Sure, she brought it on herself (and btw she may not have brought the weight on herself - some people have a hormonal imbalance that makes them gain weight even when they're eating right and exercising) but I'll grant you that she brought the "I'm not moving off this couch" bit on herself. But so what? The homeless guy on the street may very well have brought his problems on himself but that doesn't mean that you don't sympathise with the fact that he has to sleep over a heated grate in 40 below windchill this winter.
When you get right down to it, you can say people brought a LOT of problems on themselves. That might, and perhaps should, factor into your thinking about what you will do about it (I'm not giving this wino $5 because he'll just spend it on booze), but it doesn't mean you can't feel for them.
It's a little thing called compassion.
