I haven't found a scan of the form online, but The Daily Show displayed the document (I know they are a fake news source, but I'm sure this part wasn't made up):

This is a direct quote from the form. I really hope this was generated by the local people, not the party itself, since (a) 'hereby' is misspelled, and (b) 'reelection of the United States' means absolutely nothing.
I think the person who prepared the form was left behind as a child.
