Sounds to me like it's mostly mental. You're thinking that the condom's going to make you soft, and therefore what you think comes true.
Perhaps just the feeling of the condom itself has caused a subconscious reaction in your body. Regardless of the actual cause (maybe a bad previous experience? maybe just suggestion...), you either need to strongly believe that a condom will not make you soft, or find a new form of birthcontrol.

To overcome the mental aspect, I would recommend practicing wearing condoms. That is to say, plan an evening with your woman where you are going to do just foreplay and no sex. Do so with condoms. Make the goal of the evening to maintain an erection while wearing a condom. This will help retrain your brain to think it's possible to be hard while wearing a rubber.
Example: Foreplay, foreplay, erection, condom, softie. Don't remove the condom and continue with foreplay until it gets hard again. If the condom slips off, don't give up. Do foreplay until you get another erection, then reapply condom, etc.
