I can get Blue Cross/Blue Shield for about $400-500 here for a family of 5.
Blue Cross isn't the best, but they aren't bad, you just need to know how they work.
About 1 out of every 5 to 10 claims will get kicked back to you for some silly reason. You just call them straighten it out, they apologize and fix it. Never had any long-term problems with them.
That included prescription, no dental/vision (usually a seperate plan anyways) and I don't know about alternative. Offhand I couldn't tell you my deductible but the copay is $20 (for any doctor) and prescription is $5/$15/$25. This plan is a PPO.
