Not necessarily, they have aired serious injuries before, example: back a few years ago when Droz rammed into the ring post and broke his neck they kept airiing the whole thing.
I thought it was a blood capsule also but the when you see Kane's face and him going over and getting help (Kane is not that good of an actor).
Also, with works 90% of the time they have the guy getting beaten as he's going to the ambulance, and so on.

As for Refs and Slaughter one of the requirements to being a ref (from what I have been told) is that they have some emergency medical training in case something bad happens, as they are the first ones that can truly do anything.
