I also utilize the combination. Right back pocket has license, credit card, insurance card, college ID. Left back pocket has money clip.
In response to your second question,
Redlemon, I fold everything in the middle. Outside bills (the ones you see when folded) are twenties, then tens, then fives, and ones in the middle; this means ones are the easiest to procure, which works for me well enough.
