I'm not a teacher or a professor...but I'm a student and I fill out evaluation forms for each of my classes every semester. My favourite comment that I have left goes something like this:
"Professor H has no regard for personal space, and makes students uncomfortable with his occasional lewd comments. He also has a disregard for personal hygiene and appearance"
This was for my English 105 professor, who was the quintessential "Dirty Old Man"
