1. Common myth: women love to be penetrated. While most women enjoy sex, penetration isn't the part of intercourse they get off on. It's the stimulation of the clitoris that occurs from bumping and grinding. When women masturbate it's actually quite rare that a female uses penetration to get off. So, why do they portray women getting off on blowjobs/penetration? Because the target audience is men, and that's what men think women want, and what they want women to want.
3. Not only is it the cheapest lube on earth (cheap is free!), but it adds a sense of degradation to women. Since degrading women in porns sells well, the industry keeps it up. See: "Cherry Poppin' Daddies"

(This is based on lectures from my human sexuality and psychology classes.)
Fetishes can develop overnight. It
is conditioning. Sometimes it's classical, sometimes it's operant. In any case, it's possible for pairing to occur after only one experience.
Example of classical conditioning: A guy is going down on a girl. While he goes down on her, he ties her up to a bed post. As a result, the girl gets turned on at the sight of fruit.
This (cunnilingus) is a conditioned stimulus, which (hopefully) elicits a conditioned response of getting turned on. Then, you pair the conditioned stimulus with a neutral stimulus which would be something nonsexual in this scenario, let's say rope. The conditioned response is then associated with the neutral stimulus, which is then renamed to conditioned stimulus because now you no longer need the previous conditioned response to elicit being turned on.
This only has to happen once if the experience was really intense and good timing for the association to be created. However, it could take years, too. It varies for every person and their experiences.
