I know a few people here work in this industry, dont know if anyone can help with this question

For my own sake (kind of an experiment) I have been considering trying to find out all/any ghostly experiences of people I know - to try to judge unscientifically how common the perception of these things is.

I considered that if what I found was interesting it might be form the basis of something I could publish

Obviously I wouldnt want to publish anything that the person telling me about didnt want to be out there even if under a fake name (and I am sure it is far from likely it will be anything that I either will finish or anyone would want to publish if I did)

But in principle, is there any requirment to have some kind of disclaimer or allowance that people would sign if you did use an experience they had related to you in something you published yourself?

It's a good idea to get interviewees to sign a release form.

Here's one example:
Sample Information Release Form

You could customize this to the details of your project. Basically, anyone who signs it agrees to be interviewed and that the information obtained from the interview can be used by you in the publication of the book.

So long as you have concrete details: book (working) title, author's name, etc., then the forms can be presented to the publisher, or you can keep them on-hand if you're self-publishing.
