I've got an AverTV Stereo that works fine. The software leaves much to be desired, but it works fine as something to watch TV with.
I've heard good things about the ATI All-In-Wonder cards.
As for the descramblers, I think it depends on your cable system. I've seen descramblers intended to work on European cable systems, but not in the US. My particular cable company puts blocks inline with the coax in their locked box on the back of my house, so there really isn't anything passing through to descrable.
