Never happens for me, except for VERY occasionally (I can count the times on one hand, over 4 years) with the help of clitoral stimulation. And it still takes AGES to get to that point, almost making me feel like it's not worth all the effort. Our hands get tired, or the vibrator over-stimulates me, etc.
