Because they are errogenous zones. Most women enjoy having their breasts carressed (as opposed to pawed) so men have evolved to want to carress them. As for me, I am an ass-man. Breasts don't do much for me.

It's called acting. Sometimes you have to exaggerate a movement or action while you are acting so that it comes across the camera. No pun intended.

In some porn they use very small, short women so that guys dicks look bigger. The high heels are there to make the girls appear more normal sized.

Extreme fetish stuff sells. The gag thing is becoming a more usual thing though which is kind of off-putting for me. Max Hardcore was the king of this degrading act for a while.
