There is a moderate chance of her being pregnant. While the pill becomes more effective as it is taken, the reason she is supposed to wait for an entire cycle to pass is so it becomes as effective as possible. Also, just because you didn't orgasm inside of her doesn't keep you safe. Guys always have some leakage during intercourse which, unless you are sterile or have undergone a vacectomy, contains semen.
Another thing to remember...the pill is not a guarantee. Plenty of women have become pregnant while on the pill. If you are having penis to vagina intercourse, you are never fully protected from getting the woman pregnant.
