It's true that there are people who die from autoerotic asphyxiation, but it's not that common. What happens is that their need to block off air increases and gets more and more extreme in some, but not all cases of people who have this paraphilia/fetish. As their need to cut off more air increases, it gets more dangerous for them.
In the past, before they realized this was a sexual "addiction" thing, they thought they were just bizarre suicides. But they now know it's something that can be a dangerous fetish. It is mentioned in the suicide chapter of Dead Men Do Tell Tales by William Maples Ph. D. and Michael Browning, starting on page 82.
