Okay, this is not the same as How often do Fast Food places mess up your order?. For this thread, I want to know two things:

1) In what way was your order screwed up.

2) How badly was this done.

Basically, it's time to outdo everybody else!!!

So go ahead, tell us the worst it's ever gotten....scare me if you can.
