No they wouldn't. If they aren't too lazy, then they will either:
a) Have a nsfw avatar and purposely not check the box, thereby making all the people who don't want to see them see it.
-or-
b) Have a sfw avatar and check the box off, because they thought it was for something else or they just checked it off.

It is just not a simple thing for people to tag their own stuff. Its natural for things to slip by, or be done for other reasons.
