One thing to try starting with:
Anything you do that's nearsighted (tv, computer, reading), use the 20/20/20 rule: Every 20 minutes, shift your gaze at least 20 feet away for at least 20 seconds. Remember: using your vision in close range has your eyes geared up tightly; it's when we look at distances (and in darkness) that relaxes the muscles.
Other than that, there are yogic eye exercises, but I don't know how well they work. They involve repeated things such as looking up and then down, looking left and then right, doing a circular motion, and looking near and then far. Do these 10 or 12 times each exercise. Use your thumb as a close focal point for the distance exercise.
There might be a problem with your prescription though, as you hinted at. So be sure to rule that out first.
Also consider stress management. Strain can result from tension as well.
