My feeling is that "downright sick" is an (albeit overly emphatic) way of making a statement about the apparent health of the woman, whereas "fat cow" is apparently a judgement call about her personality as well as her size.
If someone said "A looks like an anorexic bitch" that would be equivalent to "C looks like a fat cow" and both would be inappropriate in my book.
I think "A looks like she's starved herself" would be equivalent to "C looks like she's gorged herself"; and I think that from my perspective, the first girl doesn't appeal to me because I am a hedonist and an Epicurean - I love to eat good food and don't think that the really thin girl would be much fun in that regard.
Also, I love the sensual feel of flesh. I love to touch a willing, sensuous and pliable woman, but somehow a really skinny girl gives me the mental image of trying to fuck a xylophone.
