Any time you tear a muscle, tendon, or ligament, the damage done is pretty much "permanent". It may heal back to normal functionality, but that does not mean it is healed to *exactly* how it was before. It is possible you are straining the old injury, even though you haven't felt any pain for a good 2-3 years, since it happened.
And the pain you feel is not necessarily coming from the testicle, rather that's just where you feel it... it's a common medical phenomenon called "referred pain"- pain felt somewhere other than where it originates.
It might also be a minor case of testicular torsion, which is a pinching or twisting of the artery that supplies a testicle, resulting in poor blood circulation, which can cause (among other things) pain. That's something to have looked at right away.
Go see your PCP right away... if they can't answer it definitively, ask for a referral to a urologist to make sure it isn't something with your genitals.
