Naw, guts aren't required, when I've (read in a book that it's been) done, it was an "in the moment" kind of thing. The plane is dimly lit, the engines are humming, it's quiet, you don't quite feel like sleeping.... soooo...
Doing it in the bathroom is what requires guts - cause that's just way up there on my nasty-o-meter... (it's all about the lighting)
